For Patients
Services

Hope Within Community Health Center is pleased to offer
general primary health care services for children and adults including
routine well-care with disease prevention screening and immunization as
indicated, women's health, non-emergent acute care, and continuity care
management of chronic medical conditions. We are unable to provide
dental services, vision care, obstetric or pregnancy care beyond an
initial evaluation, or emergency medical care. |

Eligibility Criteria
|
|
To be eligible for services, the following requirements apply:

You are medically uninsured, and
-
You reside in Dauphin or Lancaster County,
and
-
Your
income is under 250% of the Federal Poverty Level.
We are now in the process of doing
patient eligibility screening. Please complete the
eligibility application
form provided and return to:
